Form A - Confidential Medical Information Release Form for Co-Curricular Activity
This form is required for all students who are participating in the RBHS Instrumental Music Program. All students should download, complete, print, and sign Form A and submit during Registration.
This form gives consent for any approved RBHS staff/Royal Regiment chaperone to secure emergency services (medical, dental, paramedic, ambulance) for the student at the parent/guardian’s expense. Efforts will be made to contact the parent/guardian prior to treatment or hospitalization. The form also designated alternate emergency contacts.
Form B – Trip Permit / Assumption of Potential Risk and Release of Liability
This form is required at registration for all students. A new Trip Permit must be submitted each year. Note that in order to travel and perform with the Royal Regiment, students must have a current trip permit completely filled out and submitted.
This form is also required for all volunteers who will be traveling either on the uniform truck, the equipment truck, or buses to events throughout the school year. All students and parent volunteers should download, complete, print, and sign Form B and submit during registration.
The form grants permission for students to participate in extra-curricular activities with the Royal Regiment. The Assumption of Potential Risk and Release of Liability Agreement for Voluntary Activity is a PUSD form that must be completed and signed by the parent/guardian before the student can participate in RBHS Music Program events and performances. This form must be submitted every year.
Form C - DaCapo Inventory Software Acknowledgement Agreement
DaCapo Inventory is an online data management system designed for music programs to manage their enrollment, events, instrument inventory, music library, uniform inventory, and volunteer opportunities.

Volunteer Clearance Form Information
Please click on the link below if you have not been cleared through PUSD as a volunteer. We will also need a current TB test and government ID.
